Age Well at Home provides a new way to preserve homeownership by bringing housing and healthcare services together to assist older adults to remain in their home as they age.
A rapidly growing need
The vast majority of older homeowners want to live at home as long as possible. However, in-home services to help them are severely lacking. Our home modification program will help homeowners live safer and healthier in their homes.
With the rise in older homeowners, an aging housing stock and health challenges related to falls and other chronic issues, programs like Age Well at Home will make a significant impact in helping seniors stay at home longer.
What do Age Well at Home services look like?
Sometimes simple modifications like bathtub grab bars, extra railings or a higher toilet is all that’s necessary to address an older homeowners’ concerns. Other times a technology assistant like a smart doorbell, sensors or brighter lighting will create a safer home and a more confident homeowner.
Our goal is to change the home, so homeowners don’t need to change their address.
Housing and Healthcare Collaboration
Age Well at Home is a person-centered approach that brings healthcare and housing services together to collaborate with older homeowners to improve their ability to live healthier and safer in their homes.
Through our healthcare partners, patients who are prescribed and request home modifications are referred to Habitat to carry out the work. We work with homeowners to develop a custom solution that addresses their challenges and preserves their ability to stay in a healthier, safer home.
At the present time all referrals for Age Well at Home Services come from one of our healthcare partners. Currently, we partner with Allina Health and Hennepin Healthcare.
We are working to expand partnerships with all Twin Cities healthcare providers so that all income qualified older adults can access our home modification services.
